Pruebas de detección de cáncer de estómago

Pruebas de orina y de heces

Blood tests check for anemia and guide our assessment of your overall health, and stool tests can detect hidden bleeding. These tests cannot diagnose stomach cancer on their own, but they help guide your evaluation.

Biopsia

A tissue sample is taken, often through an endoscope -- a small tube with a camera -- that is inserted through the mouth and fed into the stomach.

CT, MRI, endoscopic ultrasound, and in some cases PET scan or x-rays may be used to diagnose your tumor, determine if it has spread, and evaluate your response to treatments.

Diagnostic Laparoscopy

Before treatment begins, your surgeon may use a laparoscope inserted through small incisions to look for cancer spread to the lining of the abdomen that scans can miss. This helps confirm the stage of your cancer and plan the right treatment.

Tumor (Biomarker) Testing

Your biopsy tissue is tested for markers such as HER2, PD-L1, claudin 18.2, and mismatch repair (MSI) status. These results help determine which chemotherapy, immunotherapy, or targeted treatments will work best for you.

For many patients, surgery is combined with chemotherapy and immunotherapy given before and after the operation.

Endoscopic Resection

Some very early stomach cancers can be removed through an endoscope, without any incisions.

Nearby lymph nodes are always removed at the same time to check for cancer spread, and nearby organs may also be removed if the cancer has grown into them. If your entire stomach is removed, your esophagus is connected to your small intestine so that you can continue to eat. Our nutrition team will help you adjust to eating smaller, more frequent meals.

Colocación de un stent endoluminal

When a tumor creates a blockage, your doctor may insert an expanding metal scaffolding device, called a stent, through a small lighted endoscope, to open the blockage and relieve symptoms such as nausea and vomiting.

Quimioterapia

Our medical oncologists routinely combine the newest chemotherapy drugs, which kill or slow the growth of cancer cells, with standard drug regimens to achieve a better response, fewer side effects, and improved quality of life.

For cancers that can be removed, chemotherapy is typically given both before and after surgery to shrink the tumor and lower the chance it returns. For cancer that has spread to other organs, chemotherapy is usually combined with immunotherapy or targeted therapy based on your tumor's test results.

Inmunoterapia

For many patients whose cancer can be removed, immunotherapy is now given together with chemotherapy before and after surgery, followed by immunotherapy alone. Immunotherapy helps your immune system recognize and attack cancer. For advanced cancer, immunotherapy may be combined with chemotherapy, or occasionally given by itself, depending on the results of tests on your tumor.

Radioterapia

Radiation uses focused high-energy beams to kill cancer cells. For stomach cancer, radiation is most often used to relieve symptoms such as bleeding or pain, or in select situations after surgery or when surgery isn't an option. Your care team will discuss whether radiation is right for you.

Terapias dirigidas

Depending on your tumor’s test results (for example HER2, claudin 18.2, or PD-L1), your oncologist may combine targeted therapy, which attacks specific features of cancer cells, with immunotherapy and chemotherapy to help control advanced stomach cancer and help you live longer.

Cita de paciente nuevo
O bien,

Cáncer gástrico difuso hereditario

If several relatives have had stomach cancer, or a relative was diagnosed with diffuse-type stomach cancer at a young age or with both diffuse stomach cancer and lobular breast cancer, a genetic counselor can help determine whether testing for CDH1 and other inherited gene changes is right for you. People who carry a harmful CDH1 change have a significantly increased lifetime risk of diffuse gastric cancer, a form of the disease that spreads within the stomach wall and is hard to detect until it is advanced. If you test positive for a CDH1 mutation, you may choose a preventive (risk-reducing) total gastrectomy, or annual surveillance endoscopy with many targeted and random biopsies performed at an experienced center, like Duke. Women with a CDH1 mutation also have a higher risk of lobular breast cancer.

We give chemotherapy and, when appropriate, immunotherapy, before surgery to shrink stomach cancer and treat cancer cells that may have spread. This approach has been shown in clinical trials to help patients live longer.

Líderes en cirugía robótica

Duke surgeons perform hundreds of robotic surgeries each year, including surgeries for stomach cancer. Compared with traditional open surgery, robotic surgery can mean less pain, shorter hospital stays, and faster recovery -- meaning that you move on to the next phase of treatment or return to your activities more quickly.

Terapias dirigidas

We test stomach cancers for biomarkers such as HER2, PD-L1, and claudin 18.2 to match you with the targeted therapies and immunotherapies most likely to help.

Our specialists -- medical, radiation, and surgical oncologists; gastroenterologists; radiologists; pathologists; geneticists; genetic counselors; palliative care specialists; specially trained nurses; nutritionists; and social workers -- meet regularly to discuss your care.

Acceso a estudios clínicos

Entorno reconfortante

Our Duke Cancer Center features spacious waiting areas, a Quiet Room, large infusion rooms, and a rooftop garden area.
